Yasuoka and Yasuoka traced early layouts and found evidence that telegraph operators transcribing Morse influenced the arrangement — some groupings make more sense for resolving ambiguous Morse sequences than for avoiding jams. The layout has more than one parent.

Why it never changed

Dvorak and Colemak both claim efficiency gains, and the studies are less decisive than their advocates suggest. But the real barrier is not evidence: a keyboard layout is a standard whose value is that everybody shares it. The cost of switching is borne by each person individually while the benefit only arrives if nearly everyone switches, which is a coordination problem rather than an ergonomic one.

The part that did change

Layouts did diverge by language — AZERTY in France, QWERTZ in much of central Europe — because the letter frequencies differ enough to matter. That shows the layout was never sacred, only expensive to move.

What a layout cannot fix

Most of the ergonomic argument is about finger travel, while most typing injuries are attributed to posture, force and duration. A layout change moves letters; it does not change how long somebody sits, how hard they strike, or how their wrists are held. That is the uncomfortable part of the Dvorak debate: even where the efficiency claims hold, they may be optimising the variable that matters least.

Why the number row is not in a sensible order either

It is, in fact — but only because it was easier. Early typewriters often omitted 0 and 1 entirely, expecting typists to use capital O and lowercase l, which is why those substitutions persisted in typed documents for decades. The keys that were added later went where there was room, which is also how the punctuation ended up distributed the way it is: not designed, accreted.
